This form must be faxed back and completed in full. You will then be issued with an Returns Material Authorisation Number (RMA)
The returns number must be clearly displayed on the outside of the carton.
Collection by Elite Mobile will be arranged ONLY for Goods OVER £100.
For goods under £100 it is your responsibility to arrange shipment to Elite Mobile Ltd
If you fail to supply the courier with your returns then you will be responsible for sending the returns back
to Elite Mobile Ltd.
Any returns sent without authorisation may not be credited or exchanged.
Any forms received that do not contain valid invoice numbers and account numbers will be rejected and
may not be returned.
